HIFU Cartridge Economics: The Cost Nobody Quotes
The console is quoted once. The HIFU cartridge is quoted again every month you stay in business, which is why the cheaper machine so often turns out to be the expensive one. Price the treatment, not the box.
The number on the quotation is the small one
Two suppliers send you a HIFU quotation. One console undercuts the other by a margin that looks like free money, so you sign it. Eighteen months later a consumables invoice lands that has quietly outgrown the machine, and the cheap console is now the most expensive object in your clinic.
Buyers aren't careless. The quotation format sets the trap. Consoles get compared line by line, warranty by warranty, while cartridges get a footnote and a promise of good pricing later. Yet cartridges are where the money goes, because you buy them for as long as the room earns.
So change the question. Don't ask what a cartridge costs. Ask what one completed treatment costs.
A cartridge is a depth, not a spare part
Every HIFU cartridge is a transducer built around one focal depth and one frequency, and you can't re-tune it in the field. The published instructions for use for the Ulthera System, a widely cleared microfocused ultrasound platform, list transducers at 1.5 mm and 10 MHz, 3.0 mm and 7 MHz, and 4.5 mm and 4 MHz. That same document sets energy per treatment line in joules, not as an energy density: 0.15 to 0.25 J on the 10 MHz, 1.5 mm transducer, 0.25 to 0.45 J on the 7 MHz, 3.0 mm, and 0.75 to 1.20 J on the 4 MHz, 4.5 mm, delivered as a line of discrete thermal coagulation points. Preman and colleagues, in their 2026 CosmoDerma review, put the temperature at those focal points between 60 and 70 degrees Celsius. Product literature in our engineering archive uses the same 3 mm and 4.5 mm convention for the fascial layer.
Two consequences follow straight into your budget. Covering more than one depth means owning more than one cartridge. And a cartridge is finite in two separate ways: the same instructions for use show the console reporting treatment lines remaining against the transducer's total line capacity, and warn that the expiration date printed on the transducer must be checked, because expired transducers should not be used. Shots run out. So does the calendar. Whichever comes first, you buy again.
Why one cartridge never finishes a face
Anatomy decides this, not marketing copy. The skin fundamentals training handbook in our engineering archive puts the epidermis at roughly 0.1 to 0.4 mm and the dermis at roughly 0.5 to 4 mm, with the subcutaneous layer below the dermis, connecting downward to the fascia.
Read those numbers again. The dermis alone spans an eightfold range from one patient to the next, so a fixed 4.5 mm focus is not landing in a fixed anatomical layer. It lands wherever that patient's layers happen to sit. That's why protocols stack depths instead of picking one and hoping, and why depth coverage is a clinical judgement made face by face. Your consumables bill follows that judgement, not a purchasing spreadsheet.
The arithmetic, in the order you should do it
Start with published line counts. The spread across studies is the whole story.

Now the chain, and it's short. For each depth your protocol touches, take the lines you actually use, divide by that cartridge's line capacity, and you have the fraction of a cartridge consumed. Multiply by its price. Add the depths together. That sum is your cost per treatment. Everything else is noise.
A trap sits inside it. Shots per cartridge is not a unit of area. The reference instructions for use describe a treat line adjustable from 5 to 25 mm, narrow transducers scanning 14 mm against 25 mm on the standard ones, and a worked example where a 25 mm line at 1.5 mm spacing carries 17 coagulation points. Change the assumed line length and the headline shot count moves while the hardware stays identical. Pin down line length and spacing before you let anyone put two numbers side by side.
Why the cheap console loses
Write two sums on one page. The console discount is one amount, paid once, and it never grows. The cartridge premium is charged per treatment, times every treatment you intend to sell. Multiply that premium by a realistic 24-month case volume and set it beside the discount. Four minutes of arithmetic settles the argument.
Short-life stock makes it worse in a way spreadsheets miss. Since cartridges carry a printed expiry, a slow quarter destroys inventory you've already paid for. A busy clinic burns cartridges. A quiet clinic burns them too, without the revenue attached. Ask what happens to unopened stock that ages out, and get that answer before you sign.
There's a second-order effect worth naming. When consumables are expensive, operators ration them. They shorten the map, skip the second depth, treat fewer lines than the protocol asks for, and the result drifts. Nobody writes that down. It shows up as thin word of mouth a year later.
Where the cost hides
- Expiry dating. The date is printed on the cartridge, not negotiated with your calendar.
- Depth bundles. A starter pack that mixes depths sounds generous. One depth empties first and the rest sit on the shelf until they age out.
- Line-length accounting. Shot counts computed at minimum line length are technically true and commercially useless.
- Training and test modes. The reference instructions for use note that transducer lines are not decremented while a training guideline is selected. Ask whether demo shots on your machine are free or billed to your capacity.
- Firmware pairing and regional coding. If cartridges authenticate against only one console or territory, your negotiating position is gone the day you take delivery.
- Lead time. A dead cartridge with a six-week resupply is a closed treatment room. Ask where stock sits.
What to demand in writing
We quote HIFU cartridge specifications and pricing per configuration, because depth mix, line capacity and territory all change the answer. That cuts both ways. A supplier who won't commit specifics to paper is telling you something. Put these in the purchase order, not the email thread.

Matching cartridges to the room you actually have
Cartridge economics should shape the buying decision, not just explain it afterwards. If your caseload is mostly lower face and submental laxity, you'll live on the mid and deep cartridges, so negotiate those hardest. One last item, and it belongs in the budget rather than the clinical file. The 2026 CosmoDerma review reports temporary erythema, oedema and pain as the common effects, with uncommon reactions including contact dermatitis, mandibular burns, bruising and dysesthesia, and it puts post-inflammatory hyperpigmentation risk low because melanin-rich epidermal layers are spared. Manufacturer instructions for use are blunter: contraindications include open wounds, severe or cystic acne and implants in the treatment area, the thyroid gland, major vessels and breast tissue are to be avoided, and the device has not been evaluated in pregnancy or in children. Patient selection and referral belong to a qualified clinician. Budget for the sessions you decline, the retraining you'll fund and the follow-up you give away. Those are consumables too.
