Focused Ultrasound Machines: The Categories Behind One Name
A focused ultrasound machine can mean three very different devices. You'll find surgical ablation systems, aesthetic lifting platforms, and physiotherapy warmers all sold under that name. Sorting them out starts with physics: one focuses energy, one doesn't.

What focusing actually does
Ultrasound is just sound above human hearing. A transducer emits a wave. If you shape that wave to converge at a point, you get focus. The energy passes through intervening tissue at low intensity, then spikes at the focal point. For therapeutic ultrasound, that spike can heat tissue to 60°C or more in seconds. That's the principle behind every focused ultrasound machine worth the name.
Without focus, you get diffuse heating. Physiotherapy devices use unfocused, low-intensity ultrasound since the 1930s. They warm tissue gently, maybe 1-2°C. No ablation, no coagulation points. Just warmth.
Focusing changes everything. It lets you deliver high energy at depth while sparing the skin. The beam is harmless on the way in. At the focal point, it's destructive or stimulatory, depending on intensity and pulse.
Three families, one phrase
A buyer searching for a "focused ultrasound machine" stumbles into a naming mess. The phrase covers three distinct device classes.
First, therapeutic HIFU. High intensity focused ultrasound machine. These ablate tissue. Think uterine fibroids, liver tumors, prostate. They operate under imaging: MRI or diagnostic ultrasound. The patient lies still for up to 3 hours in some protocols. The system tracks temperature in real time or uses echogenicity changes to confirm ablation. IEC 60601-2-62:2013 governs their safety. Not an aesthetic device.
Second, aesthetic microfocused ultrasound. Microfocused ultrasound (MFU) instruments heat tissue above 60°C, creating discrete thermal coagulation points (TCPs) at fixed depths: 1.5 mm, 3.0 mm, 4.5 mm. Third, low-intensity physiotherapy ultrasound. These don't focus at all. A single transducer emits an unfocused beam, heating a diffuse volume. Used for muscle strains, joint pain. No ablation, no TCPs. Often confused with focused machines because they share the word "ultrasound." They shouldn't be.
So when a clinic manager asks for a focused ultrasound machine for skin tightening, they almost always mean the second family. A HIFU surgical system would be catastrophic in that role. A physiotherapy unit would do nothing. The distinction is not subtle.
What an aesthetic platform should look like
If you're buying for aesthetics, expect fixed cartridge depths. That's the norm. A transducer cartridge is designed for one depth, say 4.5 mm for SMAS. You can't adjust it. Swap cartridges to change depth.
Delivery is line-based. The handpiece fires a sequence of TCPs along a line. Spacing is predetermined. You see a grid of tiny coagulation points under the skin, not a large ablated volume. That's the point: stimulate collagen without destroying tissue architecture.
Consumables matter. Each cartridge has a shot limit, often a few thousand lines. The machine counts shots and locks the cartridge when depleted. That's a recurring cost. Budget for it.
No imaging on most systems. You don't see the SMAS in real time. You rely on depth settings and skin thickness assessment. Some high-end systems add visualization, but many aesthetic platforms skip it. Know what you're buying.

Reading the specs without getting fooled
Spec sheets for aesthetic focused ultrasound machines list transducer depth, frequency, focal zone dimensions, and shot count. Know what each means.
Transducer depth is the distance from the skin surface to the focal point. Common depths: 1.5 mm (superficial dermis), 3.0 mm (deep dermis), 4.5 mm (SMAS). Some systems add 2.0 mm or 6.0 mm. Deeper is not always better. Match depth to the patient's skin thickness.
Frequency affects precision and penetration. Higher frequency gives a tighter focal zone but less penetration. Lower frequency goes deeper but spreads energy more. Aesthetic systems typically use 4-7 MHz. The exact value matters less than the focal zone dimensions. Ask for the -6 dB focal zone: length and diameter at the focal point. Smaller zones create more precise TCPs.
Shot count per cartridge is a cost driver. A 4.5 mm cartridge might deliver 500 lines. A full face and neck treatment uses 300-500 lines. So one cartridge per patient, roughly. Calculate cost per treatment before buying.
Check the pulse duration. Too short and you get mechanical disruption, not thermal coagulation. Too long and you risk collateral damage. Aesthetic MFU uses millisecond pulses. Safety limits that actually bind
Diagnostic ultrasound has thermal index (TI) and mechanical index (MI) limits. TI measures heating potential, MI measures cavitation risk. Obstetric scanning caps TI at 3.0, with lower values for longer scans. For example, a TI of 1.5-2.0 allows only 15 minutes. That's diagnostic imaging, not therapy.
Therapeutic focused ultrasound is regulated differently. IEC 60601-2-62:2013 covers high intensity therapeutic ultrasound equipment. It sets essential performance and safety requirements. Aesthetic MFU devices fall under this standard. Ask the manufacturer for compliance certificates.
Post-inflammatory hyperpigmentation risk is real, especially on darker skin types. Higher energy and higher density increase risk. Test on a small area first. Avoid treating active acne or inflamed skin. Refer suspicious lesions to a dermatologist before treating.

Is a focused ultrasound machine the same as HIFU?
Not exactly. HIFU is high intensity focused ultrasound, a broader category that includes surgical ablation systems. Aesthetic microfocused ultrasound is a subset. If you're buying for skin tightening, you want MFU, not a surgical HIFU.
Does a higher frequency mean better results?
No. Higher frequency gives a tighter focal zone, which can be more precise, but it also limits penetration. A 4 MHz transducer might be better for 4.5 mm depth than a 7 MHz one. Match frequency to depth and target tissue.
How many treatments will a patient need?
Most patients see results after one session, but some need two or three spaced 3-6 months apart. It depends on skin laxity and age. Maintenance treatments every 12-18 months help.
Is it safe for all skin types?
MFU is generally safe for all skin types because it doesn't target melanin. However, higher energy settings can cause post-inflammatory hyperpigmentation in darker skin. Use conservative settings and test patches first.
