Buying a Second-Hand HIFU Machine: The Risk Checklist
A second-hand HIFU machine can be a real bargain or a very expensive table with a screen on it. Three things decide which one you are getting, and none of them show up in the seller's photos. Work through this before any money moves.
Used HIFU is not the same gamble as a used laser
Buying used equipment is reasonable. Rooms open on tight budgets, and a five-year-old console still earns. But the reasons a second-hand laser is a survivable risk do not carry across to ultrasound.
With a laser you can at least see something. The beam leaves a mark, output checks against a meter, consumables are gel and goggles. HIFU gives you none of that. The energy is sound, it lands under skin you are deliberately sparing, and what you buy every month is a sealed cartridge only one company makes.
Three risks follow. Cartridge supply can be switched off by someone who is not you. Output cannot be judged by eye and rarely comes with a calibration record. The maker may want nothing to do with a unit that changed hands.
Risk one: no cartridge supply, no machine
A HIFU cartridge is a transducer built around one focal depth and one frequency, sold by the platform's maker, counted by the console. The public instructions for use for the Ulthera System, a widely cleared microfocused ultrasound platform, show the screen reporting treatment lines remaining against the transducer's total capacity, and an error reading Transducer out of lines at zero. That document is blunt about the calendar too: always check the expiration date on the transducer before using, and expired transducers should not be used.
Cartridges thrown in free with a used console may be counted down, expired, or both. Check the printed dates. Make the console show you the remaining line count for every cartridge in the box before you talk price.
Bigger question: does anyone still make them? Cartridges are proprietary and consumable, so a platform whose cartridge line has been discontinued is not a discounted machine. It is scrap. Ask the manufacturer, not the seller, in writing, with the model number in the question: are cartridges for this platform still in production, and for how long?
Handles narrow it further. On our own HF-01 4D HIFU platform, the 4D handle takes 3.0mm and 4.5mm cartridges as standard with 6, 8, 10, 13 and 16mm optional, while the V-max handle takes 1.5mm, 3.0mm and 4.5mm as standard with 8mm and 13mm optional. Most platforms work that way. A used unit arriving with one handle cannot reach the depths tied to the missing one, at any price, and the handle is the part most likely to have been dropped.
Risk two: output verification is a laboratory job
Nobody can look at a HIFU shot and tell you the power was right. The surface is meant to be spared, so the visual endpoint is thin. A machine drifting low looks exactly like one on spec, until the client returns at ninety days with nothing to show.
Measuring it properly takes a bench. IEC 62555:2013 covers power measurement for high intensity therapeutic ultrasound transducers and systems, using a radiation force balance where a target intercepts the field, or a calorimetric method based on the thermal expansion of a fluid-filled target, up to 500 W across 0.5 to 5 MHz. That is metrology, not a probe you bring to a viewing. A dedicated safety standard exists for the class as well, IEC 60601-2-62:2013. Ask which standards the unit was built and tested to, and whether the documents travelled with it.
Why push so hard on a number nobody can see? Both failure directions cost you. A weak machine sells treatments that do nothing, and refunds follow. One delivering more than the screen claims, or focusing where it should not, burns people. The clinical skin atlas in our engineering archive lists the adverse-effect set for energy-based facial treatment plainly: burns with swelling and blistering, post-inflammatory hyperpigmentation or hypopigmentation, secondary infection, folliculitis, and rare scarring where treatment is overdone, with stricter caution advised above Fitzpatrick type III. Unverified output on darker skin is the worst pairing on that page. Whether a given patient should be treated at all stays a clinician's judgement, not a setting on a used console.
Risk three: the service path may not travel with the box
This is the one that catches experienced buyers. The after-sales chapter in our device manuals conditions free warranty service on the customer holding valid proof of purchase, an invoice, receipt or contract, plus a complete and unaltered warranty card. Cover is void for damage from repair by unauthorised parties or from the owner opening the machine, and void beyond the warranty period. Ordinary clauses. They read very differently once you are the second owner and the paperwork stayed with the first. Makers say it from the other side too: the Ulthera instructions for use state the system has no user-serviceable components and that no modification is allowed.
There is a regulatory edge as well. FDA's final guidance on remanufacturing separates servicing, which returns a device to the safety and performance specifications the original manufacturer set, from remanufacturing, which significantly changes those specifications or the intended use. A workshop that rebuilt a handpiece with parts it sourced itself may have crossed that line, and what follows are a manufacturer's obligations, not a repairman's. Cheap fix, expensive category. IEC 62353:2014 covers the moment a unit goes back into service: recurrent testing and testing after repair. Have someone qualified run it, not the seller's cousin.
The inspection list
MHRA's Managing Medical Devices guidance sets a floor. Quote it at any seller. Usage and service history should always be available to prospective purchasers before sale, then supplied with the device at the point of sale. The stated minimum: reconditioning work including replacement parts, all maintenance and servicing with the name of the servicing organisation, type and frequency of usage over the working life, a fault log, and decontamination status. It also tells buyers to ask whether spares and service support are guaranteed, and for how long.

What an acceptance test should look like
Not a demo. An acceptance test: balance unpaid, machine powered, checklist in hand.
- Match the serial on the console, the handles and the paperwork, then send it to the manufacturer and wait for the answer.
- Have a qualified biomedical engineer run electrical safety to IEC 62353 before the unit goes into service. Budget for it.
- Power up from cold, open every menu, photograph the error log and any service screen. Check the case against the manual's accessories list: handles, cable, foot pedal, keys.
- Insert a cartridge of each depth you plan to sell. Confirm the console recognises both handles, and record lines remaining against capacity for every cartridge in the sale.
- Fire test shots into an appropriate load, following the coupling instructions in the manufacturer's manual. The manual is the authority there, not the seller.
- Get output verification in writing from the maker or a test house, or price the machine as though you will pay for it. You probably will.
- Then move the money.
When to walk away
- The manufacturer will not confirm the serial, or will not sell you cartridges as the new owner.
- It was opened and repaired by a third party with no record of what changed.
- The seller refuses a powered acceptance test before payment. There is no innocent version of that refusal.
- No conformity or registration paperwork valid for your market, and you would be the one placing it in service.
- The discount is smaller than one out-of-warranty handpiece. You are buying risk at a markup.
We sell new machines, so use this list on us
We are not neutral here. Pmise builds and sells new equipment, and every used console that finds a buyer is one we did not sell. Say it out loud: a checklist written to steer you is worthless, and this one is meant to be turned on any supplier, us included.
So turn it. Ask us to confirm a serial. Ask for cartridge availability and pricing in writing with a term attached, ask what our service commitment actually covers, and ask what an acceptance test looks like in your room. A new machine that fails those questions is not much safer than a used one, and the rest of our equipment buying guides take the same line on price and running costs.
Buy the used unit if it survives the list. Plenty do. Make the seller prove it instead of describing it.
